What is a Pathways Telehealth Nurse?
When you access the support of a Pathways nurse, you are accessing a health professional that:
Is a registered nurse with AHPRA
Is trained in the Pathways model of care to provide health system and disease-specific support
Is part of a larger network of Pathways nurses who can be consulted for additional expertise
Is required to maintain registration, professional competencies and ongoing professional education

What happens in a consultation?
In a consultation, the Pathways Telehealth Nurse will go through, step-by-step, your diagnosis (if you don’t have one, where you need to go to confirm/rule out a diagnosis), any treatment or management, emotional support, practical support and anything else on your mind. The Pathways nurse then works with you to develop a care plan and fill any gaps in care identified. They then help you find the services you need and guide you in how to seek referrals or access to services.
